Description: To create a custom RFI questionnaire, you can either build a new template from scratch or duplicate an existing one and modify it as needed.
Click the '' + New Template '' or '' Duplicate '' button to create a new questionnaire template. First, provide a unique template name. You will then be presented with a screen where you can create sections, add corresponding questions, and define metadata with answer types.

There are three primary answer types that can be associated with a question.
a. Paragraph.
b. Multiple Choice – A single option must be selected. Typically, the choices are Yes, No, or N/A, but any value options can be configured.
c. File Upload – This option requires the user to upload a document.
NOTE - Vendors have the option to attach a document to every question, even if the answer type is not configured as File Upload.
A question can have one or more answer types associated with it. Therefore, a question may include a combination of multiple choice and paragraph answer types. In the example below, the configured question includes a single metadata field and two answer types (multiple choice and paragraph). Typically, the Yes/No/N/A answer type is set as required, while the comment or paragraph answer type is optional. Currently, conditional logic is not supported (for example: if the answer is 'No', then require a comment).

Once the template is saved, you can start using it to send to vendors for completion.
