Description: The VQQ Request form allows Sponsors and CROs to send one or more questionnaire templates to a Vendor.  

Fields:

Vendor Name: Name of the Vendor that will be filling out questionnaires.

Contact: Select an existing vendor location of interest or create a new one not listed in our platform.  Note: you can send a questionnaire to multiple contacts at a vendor. 

Vendor's Listed Services: List of services registered on the Vendor profile in the 360 platform.  This is used to assist in determining which templates to send.

Questionnaire Templates: List of VQQ templates you want to deliver to the vendor contacts.  You can send multiple Templates at once.

Location: Vendor location of interest.  Select from a previously registered vendor location or create a new one (last entry in the dropdown)

Expected Due Date: Ideal date in which you would like to receive responses.

Additional Details: Any additional information to help the Vendor properly respond to the questionnaires, for example, through the lens of IRB services, etc). *Please note, all VQQ questions should get added to a formal template and not in this additional details field.
