Description: The RFI Request form allows Sponsors and CROs to send one or more questionnaire templates to a Vendor.  

Fields:

Vendor Name: Name of the Vendor that will be filling out questionnaires.

Primary Contact: The primary contact associated to the Vendor. This field will be auto-populated.

Questionnaire Template: List of RFI templates you want to deliver to the primary contact.

Location: Vendor location of interest.

Expected Due Date: Ideal date in which you would like to receive responses.

Additional Details: Any additional information to help the Vendor properly respond to the questionnaires, for example, through the lens of IRB services, etc). *Please note, all RFI questions should get added to a formal template and not in this additional details field.
