Description: Request a new formal assessment of a Vendor by filling out the form below.  

1. Select an existing Diligent360 Vendor or register a new Vendor.
2. Select the Assessment Location. If the primary or secondary location is not desired, add a new address from the Other option highlighted in the image below.
3. Contact - Select either the primary or secondary contact that will review the assessment request. If you need to add a new contact select the Add Contact button.

Fill out the Assessment Details include the scope (service categories, Onsite/Remote, desired draft date and additional info).
